You will play a key role in coordinating patient call and recall activity, ensuring patients are booked with the most appropriate clinician and supporting the smooth delivery of chronic disease management services across the division.

Main duties of the job

This is a patient-focused coordination role supporting the effective management of chronic disease reviews and annual health recalls across the division. You will help identify patients due for appointments, coordinate recall activity, and ensure appointments are arranged efficiently with the appropriate clinician to support high-quality patient care.

  • Contact patients to arrange initial appointments and send invitations in line with recall processes.
  • Remind patients of booked appointments for their chronic disease management reviews.
  • Support the follow-up of patient DNA appointments and help improve attendance rates.
  • Help coordinate and recall patients for annual reviews including Diabetes, Vascular, Hypertension, and Respiratory conditions.
  • Support with discharge letters and patient follow-up activity.
  • Send appointment booking links and URLs directly to patients where appropriate.
  • Ensure appointments are booked with the most appropriate clinician including HCAs, Nurses, AHPs, and GPs.
  • Work collaboratively with management and clinical teams to support efficient use of clinical time and improve patient communication.

The role requires flexibility, initiative, and commitment. You will work resourcefully as part of a wider team to ensure tasks are completed to a high standard while supporting excellent patient care and service delivery.
