This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.

Key Responsibilities

  • Clinically assess, diagnose, and prescribe (where qualified)
  • Optimise medicines and improve prescribing safety
  • Collaborate with a multidisciplinary team to enhance patient outcomes
  • Support medication reviews and structured medicines optimisation
  • Contribute to service development and innovative working practices
  • Work with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities
