This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.
Key Responsibilities
- Clinically assess, diagnose, and prescribe (where qualified)
- Optimise medicines and improve prescribing safety
- Collaborate with a multidisciplinary team to enhance patient outcomes
- Support medication reviews and structured medicines optimisation

- Work with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities
