Mocketts Wood Surgery is seeking a proactive, patient-focused Pharmacist to join our friendly multidisciplinary team in a well-established GP practice serving approximately 9,400 patients in Broadstairs, Kent. This is an opportunity to contribute to high-quality primary care, medicines optimisation, and prescribing safety within a supportive, forward-thinking environment. The successful candidate will work closely with GPs, Nurses and the wider clinical team to improve patient outcomes, support safe and effective prescribing, and contribute to practice quality improvement and compliance.

Main duties of the job

  • Leading prescriptions team
  • Undertake structured medication reviews (SMRs) and medicines reconciliation
  • Manage repeat prescribing systems and support safe prescribing processes
  • Provide expert medicines advice to clinicians and patients
  • Support patients with long-term conditions (e.g. CVD prevention, diabetes, asthma and hypertension)
  • Conduct prescribing audits and contribute to QOF and quality improvement work
  • Work closely with medicines optimisation team/ICB to identify and reduce medicines waste and prescribing risks
  • Respond to medication queries from patients and community pharmacies
  • Contribute to CQC standards through medicines management governance, audits and documentation compliance

We provide care to approximately 9,400 patients and have a supportive multidisciplinary team including 5 GPs, 3 Nurses, a Nurse associate, 2 HCAs, a physiotherapist, Diabetes Nurses and administrative staff. Job responsibilities

  • Clinical Practice
  • Undertake Structured Medication Reviews (SMRs) for patients with complex needs, polypharmacy, or high-risk prescribing
  • Perform medicines reconciliation post-discharge and following clinic letters to ensure safe continuity of care
  • Provide specialist pharmacological advice to clinicians and support evidence-based prescribing decisions
  • Manage and respond to medication-related queries from patients and external healthcare providers
  • Oversee and continuously improve repeat prescribing processes to ensure safety, efficiency, and compliance
  • Lead the prescriptions team, providing supervision, training, and support
  • Identify and mitigate prescribing risks, implementing appropriate safety measures

    Person Specification

    • Experience
    • GPHC-registered with independent prescribing qualification
    • CPPE completed or ongoing
    • Confident with EMIS
    • Strong clinical knowledge and commitment to evidence-based practice
    • Excellent communication and teamwork skills
    • Ability to work independently within a primary care setting
    • Experience in medication reviews and medicines optimisation
    • Independent Prescriber qualification with specialist interest in cardiovascular health
    • Previous GP Practice or PCN experience
    • Understanding of CQC standards and primary care governance
    • Experience in audits, QOF, or quality improvement work
