Minster Medical Practice is seeking an additional highly motivated, professional, caring and enthusiastic Advanced Nurse/Clinical Practitioner to work within our established team. We are a SystmOne practice operating a very successful total triage system using SystmConnect.

Main duties of the job:
The successful candidate will act as an autonomous practitioner working independently (well supported) and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care and initiating internal and external referrals as appropriate. GP and ANP/ACP colleagues provide opportunities for education, advice and support.

Job responsibilities:
- Independently diagnose, treat, and manage patients presenting with undifferentiated and differentiated conditions
- Manage acute, same-day, and routine presentations in line with agreed clinical scope and competencies
- Prescribe medications appropriately as an independent prescriber
- Request, interpret, and act upon diagnostic tests and investigations
- Provide follow-up care, safety-netting, and onward referrals as appropriate
Person Specification:
Experience:
- Significant post-registration clinical experience, ideally in primary care.
- Ability to work autonomously and manage clinical risk
- Excellent communication and interpersonal skills
- Commitment to patient-centred care and teamwork
Qualifications:
- Current registration with a relevant UK professional body eg NMC
- Master's degree in Advanced Clinical Practice
