Mini Med is seeking a Patient Services Representative in Proactive Services to help improve daily management for people with diabetes.

You will lead proactive outbound outreach from onboarding coaching through the therapy journey, building confidence and loyalty while coordinating with internal teams.

Responsibilities include guiding patients and carers to education and self-service tools, contributing to program improvements, and managing outreach and administrative tasks to meet KPIs.
