Role Background:

The Enhanced Access Support Worker plays a vital role in delivering early intervention mental health support to individuals aged 11 and above across the Bradford and Craven District. This service is designed to provide accessible, short-term, one-on-one telephone support sessions for people experiencing mild to moderate mental health challenges. Wellbeing practitioners offer tailored guidance and practical advice to help individuals improve and maintain their emotional well-being. Each session is person-centred, focusing on the unique needs of the individual, and may include signposting to longer-term services or community resources.

The role requires empathy, active listening, and the ability to build rapport quickly over the phone. Support Workers must be confident in assessing needs, providing appropriate interventions, and maintaining accurate records. They are part of a wider mental health support network and contribute to the overall goal of reducing barriers to mental health care and promoting early help within the community.

Role Purpose:

We’re seeking to appoint passionate and enthusiastic individuals to join our team of practitioners working within our Enhanced Access service. Our Enhanced Access service provides 1-on-1 telephone support sessions for individuals facing mild to moderate mental health challenges. This service is available to those registered with GP practices in Bradford and Craven. The service operates between Monday to Saturday. Any shifts on Monday through to Friday are 3.5 hours long running from 18:15 to 21:45, and any weekend shifts are 4.5 hours long running from 10:00 to 14:30, during which several people (aged 11+) receive support sessions focused on early intervention. The support is tailored to each person’s needs, helping them make positive changes to improve and maintain their well-being. This can include offering practical advice, guidance, or referrals to long-term support services within the Bradford and Craven District.

Role Outcomes:

  • Provide 1:1 telephone support, including developing tailored support plans and completing relevant signposting/onward referrals to enable people to stay well
  • Have a good understanding of support services across the district and signpost/complete onward referrals to relevant services
  • Complete risk assessments and incident reports, where appropriate
  • Record information electronically and maintain up to date records
  • Work effectively with colleagues
  • Attend Monthly team meetings and organisational days to stay informed of key information and policies
  • Ensure that in all aspects of the role you embody the aims, objectives, and core values of Bradford District and Craven Mind
  • Ensure that the relevant colleagues have a competent understanding of, and comply with, all Bradford District and Craven Mind policies and procedures
  • Take responsibility, with colleagues, for ensuring that all Health and Safety, Safeguarding, Information Governance and Equality & Diversity requirements are met, and all other Company policies complied with
  • Undertake any other reasonable duties or tasks deemed necessary by the senior management team
