Mind in the City, Hackney and Waltham Forest (Mind CHWF), in partnership with East London NHS Foundation Trust (ELFT) and local voluntary sector providers, is recruiting for a Community Connector. You will support people with complex mental health and social challenges to access personalised, community-based support, identifying goals and connecting them to services and activities that promote wellbeing, recovery and social inclusion.
