Bradford District and Craven Mind is seeking a Health Engagement Practitioner to support people living with Severe Mental Illness (SMI) through our Physical Health Checks (PHC) service. The role is hybrid, combining home or office work with GP practice visits, with occasional home visits carried out in pairs with Health Engagement Officers.
You will empower clients through person-centred care to improve participation in PHCs and onward interventions, build strong relationships with Primary Care.
