Weâre recruiting a Rehabilitation Lead to join our Home First Reablement Team at Milton Keynes City Council. Our Home First Reablement Team supports people referred from hospital and the community to recover, rebuild confidence and regain independence at home for up to six weeks. This vital service plays a key role in keeping the flow between hospital and community moving, helping people return home safely and reducing the need for longâterm care packages wherever possible.
In this role, youâll provide therapyâled, personâcentred rehabilitation across hospital wards, peopleâs homes and community settings. Youâll work closely with health and social care colleagues to support recovery, mobility and independence, while also overseeing and supporting a small team of rehabilitation support workers. Main Responsibility
- Delivering therapyâled, personâcentred rehabilitation to people in hospital and community settings, supporting recovery, movement and independence.
- Supporting hospital inâreach work, helping people maintain and improve mobility and preparing them for discharge home.
- Overseeing the dayâtoâday work of rehabilitation support workers, providing guidance, supervision and support to maintain high standards of care.
- Completing assessments, reviews, risk assessments and support plans, ensuring records are accurate and up to date.
- Working closely with health and social care colleagues to respond to changing needs, including crisis situations.
- Safeguarding vulnerable adults and working in line with council policies, procedures and professional standards.
The Ideal Candidate
- Have experience supporting vulnerable adults with recovery, rehabilitation or reablement needs.
- Are confident working across hospital and community settings and supporting people with a range of needs.
- Communicate clearly and sensitively, both verbally and in writing.
- Understand the importance of personâcentred, strengthâbased approaches to care and rehabilitation.
- Are organised, adaptable and able to respond calmly in complex or changing situations.
- Can work flexibly across rotas, including early mornings, evenings and different locations across Milton Keynes.
Full UK Driving Licence and access to a vehicle
To be successful in this role, youâll need a full UK driving licence and access to a vehicle. If we offer you the role, weâll ask you to provide evidence of your driving licence and vehicle insurance, including cover for business use. Weâre unable to confirm an appointment until these documents are received. As part of the pre-employment checks, should you be offered the role, weâll carry out an Enhanced Disclosure and Barring Service (DBS) check, (unless you are currently registered with the DBS Update Service).

Our roles are either designated âsite basedâ or âhome basedâ. âSite basedâ roles can include up to two days a week working from home whilst âhome basedâ roles are based at home four days a week. Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
