We’re recruiting two Rehabilitation Support Workers to join our Home1st Reablement Service. The Home1st Reablement Service supports individuals discharged from hospital, as well as those living in the community, who require rehabilitation, care, and support for a period of up to six weeks. The rehabilitation support workers provide frontline care within people’s homes and play a vital role in delivering high-quality, person-centred support that promotes independence and recovery.

You’ll be part of a dedicated team that helps people rebuild confidence, regain daily living skills, and stay as independent as possible. This is a rewarding role where your care, patience, and practical support can make every day matter for people across Milton Keynes. You’ll work in people’s homes and may also support people in other settings, including hospital, depending on service need.

We’re looking for someone who can work safely, communicate clearly, and treat every person with dignity and respect. You’ll also work closely with colleagues, families, therapists, coordinators, and partner organisations to provide joined-up support. Working Pattern

The successful candidate will work on a rolling four-week rota, averaging 36 hours per week. The pattern includes a combination of AM and PM shifts, with hours spread across the four-week cycle. Shift lengths are typically 6 or 6.5 hours, with one shorter 5-hour shift included in the rota.

Key Responsibilities

  • Deliver high-quality recovery and rehabilitation support to people with a range of care needs.
  • Record clear and accurate information using council systems, care records, electronic duty notes, work logs, and electronic medication records.
  • Complete mandatory training and use your learning to improve your own practice and support service improvement.
  • Respond safely to changes in a person’s needs, including concerns about health, mobility, safeguarding, or risk.
  • Encourage people to do as much as they can for themselves, while recognising when extra help is needed.
  • Provide personal care with dignity, care, and attention to infection control, moving and handling, and safe use of equipment.

Ideal Candidate

  • Access to your own transport is essential, as you’ll need to travel across different locations in Milton Keynes.
  • You bring an understanding of the needs of vulnerable people and a respectful approach to supporting independence.
  • Your ability to communicate clearly will help you build trust, record information accurately, and share updates with the right people.
  • Experience in care, support, rehabilitation, or frontline public service would help you settle into the role.
  • A strong understanding of confidentiality will help you handle personal information safely and appropriately.
  • Confidence when responding to complex or crisis situations will help you assess risk and seek guidance when needed.
  • An approach that demonstrates dedication, respect, and collaboration will help you support people in a way that feels safe, kind, and empowering.
  • Full UK Driving Licence and access to a vehicle.

As part of the pre-employment checks, should you be offered the role, we’ll carry out an Enhanced Disclosure and Barring Service (DBS) check, unless you are currently registered with the DBS Update Service.
