Main duties of the job: Are you looking for a practice that actually invests in you? We are looking for a brilliant nurse to join our multidisciplinary family. Whether you are an experienced Practice Nurse who loves the variety of general practice based care, or an ANP/Nurse Prescriber who thrives on acute diagnostics, we are interested to hear from you.

  • If you're a practice nurse looking for your next move, you'll be the heart of our long-term condition management working alongside other experienced practice nurses and health care assistants. From immunisations to cytology and chronic disease reviews, you'll build lasting relationships with our diverse patient population.
  • If you are an ANP or a Nurse Prescriber, you'll be our go-to expert for on-the-day acute presentations along with a team of other clinicians including GPs, Pharmacists, ANPs and Paramedics. You'll use your advanced diagnostic skills to assess, treat, and prescribe, providing fast relief for patients when they need it most.
  • Want to specialise in Diabetes? Respiratory? Minor Illness? We don't just "allow" training; we push for it. We love seeing our nurses evolve.

About us: Millgate Healthcare Partnership serves the diverse community of Denton, a town within the Tameside Metropolitan Borough in Greater Manchester. Denton offers a mix of urban and suburban living, with good transport links to Manchester city centre and surrounding areas. We use EMIS and are committed to continuous improvement and development. Working closely with our PCN means you're surrounded by Pharmacists, Physios, and Social Prescribers. Job responsibilities: Whether you are managing complex acute cases or supporting a patient through a long-term condition, you'll be a vital part of our 21k-strong patient community. This is a role for someone who loves variety, autonomy, and making a real difference. The role will clearly depend and be moulded to your experience and qualifications.

  • Assess, diagnose, and treat a diverse range of acute and chronic conditions. Whether it's an on-the-day presentation or a complex review, your clinical judgment is key.
  • Lead the way in everything from wound care and immunisations to travel medicine and health promotion.
  • Request, process, and interpret pathology results, ensuring our patients get the right answers at the right time.
  • Develop and implement individualised care plans for patients with long-term conditions. You aren't just treating symptoms; you're preventing adverse effects and improving lives.
  • Help us smash our QOF and practice targets while ensuring our chronic disease registers and call/recall systems are top-notch.
  • We are a multidisciplinary powerhouse. You'll mentor students, support newly qualified staff, and perhaps get involved in Clinical Audit.
  • Work closely with our PCN and external agencies to ensure our most vulnerable patients never fall through the cracks.
  • From safeguarding to clinical emergencies, you are a calm head in a busy (but friendly!) environment.

Person Specification:

  • Experience of working in a primary care environment.
  • Experience of working as a practice nurse.
  • Disclosing Barring Service (DBS) Check.
  • Occupational Health Clearance.
  • Meet the requirements and produce evidence for nurse revalidation.
  • Evidence of continuing professional development (CPD) commensurate with the role of an ANP.
  • Access to own transport and ability to travel across locality on a regular basis.
  • Flexibility to work outside core office hours.

Qualifications:

  • Registered Nurse with Nursing and Midwifery Council.
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
  • Clinical knowledge and skills in chronic disease management.
  • Clinical knowledge and skills including: ECGs, Venepuncture, New patient medicals, Immunisations (routine and childhood), Women's health (cervical cytology, contraception, etc.).
  • Understand the importance of evidence-based practice.
  • Broad knowledge of clinical governance.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understand when to refer to GPs.
  • Knowledge of health promotion strategies.
  • Understand the requirement for PGDs and associated policy.
  • Polite and confident, flexible and cooperative.
  • Motivated, forward thinker.
  • Problem solver with the ability to process information accurately and effectively, interpreting data as required.
  • High levels of integrity and loyalty.
  • Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations.
  • Diabetes management expertise (though training can be provided).
