This is an exciting opportunity to work across two specialist transitional Supported Living services in Bristol and South Glos. Help us in our mission to support people with learning disabilities and/or autism to live their best life.

Graeme Close is in the popular Fishponds area and Blackhorse House is in Downend. Both services are unique move-on services for young people with learning disabilities and autism, or those slightly older who are living away from home for the first time. Both services provide a structured and welcoming environment that is designed to give people with learning disabilities and/or autism the space and support they need to gain skills, confidence and independence needed to flourish and transition into a more permanent setting.

We’re looking for a driven and proactive person to lead a team, demonstrating a truly person-centred approach and the desire to empower others. You will need to lead a team to effectively support individuals to achieve outcomes and enable them to transition to independent living.

The focus of both services is transition. We aim to work collaboratively with people so that they develop strategies to manage their feelings and behaviours, learn skills which mean they are more independent and need less support, and make the most of opportunities to have meaningful roles and relationships – in short, to have a life.

You’ll be responsible for leading, managing and inspiring Personal Assistants who will provide 24-hour support to the tenants at the services, through ensuring all support plans, risk assessments and records are regularly reviewed and relevant, as well as leading team and 1:1 meetings with your staff.

This is an exciting opportunity for someone with care management experience to start something new and have a positive impact on the lives of the people we support.

The successful Service Coordinator will:

  • Be able to inspire and lead a team, demonstrating excellent people-management skills together with enthusiasm and dedication.
  • Be passionate about citizen rights, inclusion and community, with an understanding of the Mental Capacity Act, Safeguarding Adults, and other relevant legislation.
  • Be an excellent communicator, confident liaising with families, professionals and other agencies involved in peoples' lives.
  • Be an excellent organiser, able to juggle competing priorities.
  • Be creative and resourceful in your approach.
