Healthcare Support Worker (Band 3)

We are seeking to recruit an enthusiastic and highly motivated Community Healthcare Support Worker to join our South Staffordshire Community Learning Disability Team, covering the South Staffordshire area. We have bases in both Lichfield and Stafford.

We are an award winning service and rapidly expanding to meet the needs of people with learning disabilities in the community. You would work as part of a multi-professional team consisting of Nurses, Occupational Therapists, Physiotherapists, Psychiatrists, Psychologists and Speech and Language Therapists.

You will work collaboratively with the Specialist Community Learning Disability Team, mainstream services and statutory and Independent Sector Services.

You will visit clients in their own homes, in day services, in residential care homes or in other community settings.

This post includes 2/3 days working at Queens Hospital supporting the Acute Nurse Liaison with reasonable adjustments for service users attending outpatient appointments or who are admitted. 2/3 days a week will be supporting service users in the community and may include those that are attending outpatient appointments at Queens Hospital or County Hospital.

Previous experience of working with people with learning disabilities in the community is essential. The post holder will need to be able to meet the travelling requirements of the post and access to a car is essential., This is an exciting opportunity for anyone that has a passion for supporting people with learning disabilities in the community.

JOB PURPOSE:

To work as part of the Community Learning Disabilities Team for the South Staffordshire area.

The post holder will work in a person centred way providing support to registered staff (Nurses, Psychologists and Occupational Therapists) working with adults with learning disabilities and complex healthcare needs. This may involve carrying out specific health focussed activities, observing behaviour or gaining information from staff or families. Information gained will play an important part in the formulation of the difficulties experienced by the service user.

The post requires travel across the area covered by the community team. This post will require the post holder to be able to work in Queens Hospital, Burton 2/3 days a week supporting people with a learning disability to ensure that reasonable adjustments are supported during their stay. This will be under the guidance of the Acute Liaison Nurse.

To carry out clinical activities as requested by health care professionals within the community team. All clinical work will be agreed with, planned (based on a clinical formulation) and executed under the supervision of the appropriate health care professional. All clinical activities to be documented clearly in the care plan with supervision provided by the health care professional responsible for the care plan.

1. To work in Queens Hospital 2 days a week supporting reasonable adjustments to be put in place for service users attending the hospital. This could be those attending an outpatient appointment or those that are admitted.

2. Where required support service users with hospital appointments at County Hospital, Stafford and Queens Hospital, Burton.

3. To work with service users who have very complex needs which may include severe learning disabilities, autism or additional mental illness.

4. To contribute to the overall assessment, planning, implementation and review of individual service users and to lead specific assessments under the supervision of qualified staff.

5. To be part of a person centred service where the needs of the individual will determine the type of intervention. The post holder will work across a variety of settings which are relevant to the service user.

6. To carry out work in the community sometimes in conjunction with other members of the multi-disciplinary team.

7. To complete physical health investigation tasks such as pulse, blood pressure monitoring, temperature, ECG’s and venepuncture.

8. To work as part of a team and in conjunction with other professionals, statutory agencies and any associated service providers for the service user. This may include the Intensive Support Team.

9. To develop a professional relationship conducive to the client reaching their full potential, within stated goals.

10. To be involved in relevant meetings/reviews related to the service user’s treatment/service.

11. To receive clinical and managerial supervision to maintain best practice. This will include discussion of caseload, reflective practice and the ability to receive and act upon constructive feedback. Clinical supervision should be received from relevant professionals in relation to caseload.

12. To take part in training and development as agreed at appraisal. To undertake own continuing professional development by demonstrating an ongoing reflective learning style in the workplace. This should be clearly and regularly documented in a personal portfolio.

13. To participate in all mandatory and statutory training deemed necessary for this role in line with the Trust’s training matrix.

14. To use an appropriate range of teaching media to assist with clinical interventions, teaching other staff, carers and service users, students and other disciplines where appropriate.

15. To ensure that all Trust policies and procedures are implemented and adhered to within the area of practice.

16. To undertake other associated duties linked to the job and grade, to reflect the evolving nature of the service.

17. To assist in the development of a flexible service which may include evenings and weekends. (This will be agreed between manager and worker)

18. To adhere to Trust and professional guidelines on confidentiality and to communicate any issues to the team leader.

19. To be responsible for own and others health and safety, reporting all issues to the line manager and working within the Trust’s health and safety policies and the Health and Safety at Work Act.

20. To participate in own appraisal by completing preparation documentation and actively partaking in the appraisal process.

21. To respect the individuality, values, culture, religion and diversity of clients contributing to a service to meet their needs.

22. Participate in local, national and organisational clinical governance activities as required including audits, surveys, evidence-based practice and research projects…
