Overview

Midlands Partnership NHS Foundation Trust is seeking a Community Support Worker in the Older Adult Community Mental Health Team in Telford. The closing date is 12 November 2025.

Following latest Home Office guidelines and legislations, it is not possible for visa sponsorship to be provided to Band 2 and Band 3 roles. If it is identified that you require sponsorship to undertake this role, your application may be withdrawn.

Do you have experience of working within mental health services specifically with older people with dementia? We are seeking a compassionate, motivated and proactive Mental Health Support Worker to join our Older Adult Community Mental Health Team in Telford. This is an exciting opportunity to play a vital role in supporting older people with complex mental health needs, enabling them to maintain independence and quality of life within their community.

Working under the supervision of registered professionals, you will contribute to the delivery of safe, person-centred care and support service users experiencing functional / organic mental health conditions, such as dementia.

Main duties of the job

  • Provide practical and emotional support to older adults in the community, promoting recovery, wellbeing and independence.
  • Support individuals in developing coping strategies, maintaining daily living skills, and engaging in meaningful activities.
  • Assist with monitoring mental state, physical health and risk, reporting any concerns to the clinical team.
  • Participate in care planning and review processes alongside registered practitioners.
  • Work collaboratively with families, carers, and multidisciplinary colleagues to deliver holistic care.
  • Maintain accurate and timely documentation in electronic records systems.
  • Contribute to creating a safe, inclusive and recovery-focused environment for all service users.

Person Specification

Experience

  • Advanced Apprenticeship in (Mental) health and Social Care
  • NVQ 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSE\’s or Functional Skills)
  • Evidence of working within mental health services specifically with older people with dementia
  • Expereince of working in a community setting
  • Experience in working with patients with Dementia at various stages.

Skills

  • Demonstrate understanding of Equality and anti-discriminatory practice
  • Knowledge of the wide range of statutory, independent and third sector service provision, for older adults with mental health problems
  • Range of Interpersonal skills, (communication, listening, working collaboratively) maintaining good relationships with other service users/carers, service providers and other agencies.
  • Knowledge of the National Dementia Strategy and NICE Guidelines on Dementia.
