An exciting opportunity has arisen within the critical care outreach team for a dynamic, innovative and enthusiastic individual who will take on the role of a band 6 critical care outreach practitioner. We are looking for a self‑motivated, adaptable and knowledgeable registered nurse who is clinically confident, able to work independently yet as part of a team. The successful candidate should have excellent communication skills, strong decision‑making ability and clinical expertise, together with a drive and passion for improving the way healthcare is provided. Main Duties

  • The successful candidate will be an experienced critical care nurse with a high level of knowledge surrounding the deteriorating patient and will drive early recognition and management of the deteriorating patient at ward level.
  • They will be part of a specialist nursing team providing a 24‑hour, 7‑day service and will be required to work flexibly.
  • Excellent communication skills are essential to facilitate the effective multidisciplinary approach in the recognition, escalation and management of deteriorating patients across the Trust.

Detailed Responsibilities

  • Compassionate person‑centred care: Provide specialist nursing care for deteriorating/high‑risk patients, including Level 3 transfers and follow‑up services for patients discharged from ICCU and those with tracheostomies.
  • Evidence‑based, clinically effective practice: Plan and deliver care aligned with professional values of care, compassion and respect, and act as a change agent to develop clinically effective practice through evidence‑based implementation.
  • Continuous quality improvement: Support the Senior Sister / Charge Nurse in fostering a culture of protecting patients from harm, ensuring a safe and effective service with continuous quality improvement.
  • Patient safety: Promote a clean and safe environment, ensure compliance with legislation, policies and protocols, maintain high standards of basic care and record keeping, report incidents and near misses, and contribute to the development and implementation of evidence‑based protocols.
  • Patient journey coordination: Plan and coordinate the episode of care, ensuring smooth transitions, safe Level 3 transfers and effective communication with interdisciplinary teams.
  • Clinical expertise and advocacy: Advise on clinical decisions, monitor quality of patient care, act as an advocate for patients, negotiate safe care with ward staff, and support a culture of person‑centred care.
  • Leadership and role modelling: Act as a visible role model, promote a supportive ethos, assist in staff development, participate in appraisal processes, and support continuous improvement cycles.
  • Learning and development: Support learning and development of junior staff, facilitate orientation and induction programs, and engage in teaching programmes such as ALERT.
  • Practice setting management: Delegate care appropriately, manage the nursing/midwifery team in absence of senior nursing staff, participate in recruitment, performance and sickness management, and support E‑rostering.
  • Networking and service development: Contribute to the development of an organisational culture that supports continuous learning, evidence‑based practice, succession planning, and participate in professional forums.

  • Essential criteria: NMC Registration, Nurse Mentorship / Appropriate teaching qualification, Post‑registration ENB100/Equivalent Critical Care Post‑Registration Course (15 months), Evidence of professional updating within last 12 months.
  • Desirable criteria: Health Assessment Qualification, Data audit and evaluation experience.
