Health Care Support Worker – Community Nursing

Job Overview: We have an exciting opportunity for a Health Care Support Worker to join our community District Nursing team in Shelton & Hanley PCN. This is a full time, permanent role based at Bentilee Neighbourhood Centre. We are a passionate team who strives to improve quality of health care for our patients.

You will be part of a team striving to achieve the best in clinical care. The service will deliver a pathway of care in accordance with the latest evidence-based practice, which puts the service user’s needs at the centre of the service.

We operate a 7-day service from 08:00 to 18:00. Due to the nature of community working, ability to travel across the patch to provide care to patients is essential; access to a car during working hours is required.

Main duties of the job

  • Adhere to the Care Certificate standards and all other relevant guidance from the Partnership Trust. Be aware of Nursing & Midwifery Council (NMC) guidance/codes of conduct.
  • Establish and maintain effective working relationships within the Partnership Trust, with external organisations and our patients to meet service requirements and support equality values.
  • Contribute to the treatment and care for individuals, families and carers under the guidance of a registered practitioner. Care/support plans may include the following:
  • Promoting independence/self-management and maintaining safety
  • Diabetic care, including insulin administration and blood glucose monitoring
  • Medication administration and injections
  • Wound care
  • Moving & Handling, including moving patients
  • Observations, venipuncture
  • Catheter management
  • Admission avoidance
  • Specific skills as listed in the person specification

All care is delivered under the guidance of the registered nursing team. You will consider each service user’s personal circumstances and preferences to implement and adjust programmes of care and intervention.
