We have an exciting role on offer within our Adult Social Care and Health Service as a Hospital to Home Response Officer. You would become part of an ambitious team delivering an invaluable service to the residents of Middlesbrough. The role involves supporting people to live at home independently for longer and enabling safe and speedy discharge from hospital.

Responsibilities

  • Install essential equipment and provide services required to enable a safe and speedy discharge and to help maintain independence within the service user’s home.
  • Provide a high standard of person-centred care when responding to emergency situations.
  • Assess service user needs, offer support, and decide on the most appropriate action to be taken.
  • Work alongside Connect Customer Advisors, Connect Response Officers and other associated professionals to provide a service throughout the Middlesbrough Area.

Qualifications and Requirements

  • Training in emergency first aid, moving and handling, and use of all lifting equipment.
  • Technologically confident with good communication skills; ability to respond with empathy and provide reassurance to service users.
  • Empathetic, patient, and passionate about supporting vulnerable people.
  • Organised with excellent communication skills.
  • This post is on a shift pattern which includes weekends and late nights.
  • Full UK Manual Driving Licence is essential for this role.
