We are seeking a Customer Support and Advice Worker for our Adult Social Care and Health Integration team. This role is part of the newly developed ACT (Accessing Change Together) service, which integrates council staff and partner organisations to provide housing, homelessness, domestic abuse and substance misuse support. As a member of one of our interconnecting teams – Front Door, Housing Solutions, or Community Interventions – you will work within a multi‑disciplinary approach to help service users access the right support.

Responsibilities

  • Assist in developing risk assessments and support plans for service users.
  • Work collaboratively with Adult and Children’s Social Care, Public Health, and partner organisations.
  • Participate in integrated case load management using a shared system.
  • Provide care and advice to individuals seeking housing, homelessness, or domestic abuse assistance.
  • Support the team in developing consistent pathways and effective processes.

Qualifications

  • Experience working effectively with a range of services.
  • Demonstrated skills, passion, and resilience to work creatively with service users.
  • Ability to respond quickly to multi‑disciplinary needs.
  • Previous experience in housing, homelessness or domestic abuse support is desirable.
