About The Role

An exciting opportunity has become available for a Respiratory Nurse Specialist to join PERT (Pinderfields Emergency Respiratory Team).

We are team of Respiratory Specialist Nurses, Advanced Clinical Practitioners, Respiratory Physiotherapists, Advanced Clinical Support Workers & Administrative Co-Ordinators. The service operates 7 days per week including Bank Holidays and provides service cover until 8pm.

The team is responsible for the Respiratory Virtual Ward and Home Ventilation Service for Mid Yorkshire Teaching Trust.

As part of the role of the Respiratory Nurse Specialist you will be expected cover and provide in-patient and community reviews.

The team is based primarily at Pinderfields General Hospital with cross-site working as required and the Respiratory Virtual Ward covers both the Wakefield and North Kirklees district.

Previous experience within Respiratory is desirable but not essential, We would consider appointing the right candidate into a Band 5 Development post.

Main duties

The outreach respiratory team aims to support the following objectives: Admission Avoidance for patients with an exacerbation of COPD, Early Supported Discharge Scheme, Home Mechanical Ventilation, In patient oxygen assessments, ward based asthma reviews and specialist respiratory ward based patient reviews.

As a band 6 respiratory specialist nurse you will undertake all aspects of the role using advanced clinical assessment skills and respiratory evidence based knowledge. The role will involve both hospital and community based assessments and you will be required to work trust wide from any of our 3 hospital sites.

This will be achieved through the core dimensions of the role, clinical practice, and acting as the patient advocate and through audit and education.

Key Requirements

  • Will undertake specialist respiratory assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills.
  • Identify suitable patients for early supported discharge, plan and arrange discharge and provide individualised, holistic care plans to suit the patient’s needs.
  • Review patients in the home environment following referral for admission avoidance. You will undertake advanced clinical assessment obtaining a history, clinical examination and conclude diagnosis implementing a treatment plan. You will also make the judgement if the patient can safely be managed at home or if they require hospital admission.
  • Review patients in accident and emergency or ambulatory care unit for admission avoidance following an initial assessment by the medical team. If deemed suitable for home care you will plan and implement the patients discharge and arrange follow up at home.
  • You will undertake frequent home visits to patients who are receiving an acute episode of PERT care ensuring their clinical condition is stable, maintaining a safe alternative to in -patient hospital care.
  • You will ensure all documentation and communication is maintained to a high standard.
  • You will liaise with members of the multi-disciplinary team and refer to other services and health care professionals where appropriate ensuring holistic care.
  • Manage a patient caseload of those requiring long term non- invasive ventilation providing expert assessment, planning and evaluation.
  • You will provide specialist respiratory nurse/respiratory education to patients on the wards undertaking in-patient asthma reviews, COPD management, inhaler optimisation and oxygen prescribing advice.
  • You will perform in-patient oxygen assessments in accordance with best practice guidelines.
  • You will routinely perform arterial blood gas analysis and other investigations such as Tosca monitoring within the patient’s own home.
  • Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
  • Participate in specialist outpatient clinics such as the home ventilation clinic and MDT MND clinic.
  • Establish and maintain effective communication with patients and carers / relatives.
  • Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
  • Recognise changes in patient’s condition that require the intervention of others and refer on as appropriate.
  • When appropriate you will be expected to undertake a non-medical supplementary and independent prescribing qualification in order to prescribe within area of speciality for your patients.
  • Work with the CNS / Team Leader to promote the development of nurses undertaking extended roles as considered appropriate.
