Join Our Team as a Community Nursing Team Leader Wakefield Integrated Care Team Are you a passionate and driven nursing professional looking for a new challenge? An exciting and rare opportunity has arisen to join the Integrated Care Team (ICT) within Wakefield Community Services, part of the Mid Yorkshire Teaching NHS Trust. Were seeking an enthusiastic, compassionate, and innovative leader to help shape the future of adult community nursing. This is a unique opportunity to provide both operational and clinical leadership to a dedicated team of nurses and healthcare support workers.

What Youll Do

  • Lead by example, inspiring your team through compassionate care and professional excellence.
  • Provide line management and support to qualified and unqualified nursing staff.
  • Offer clinical expertise in complex situations and support staff during challenging visits.
  • Drive service development and team innovation, always keeping patient care at the heart of everything we do.
  • Collaborate closely with nursing and therapy colleagues across Adult Community Services. Provide visible leadership and line management for the Integrated Care Team (ICT), supporting other teams when required.
  • Offer clinical expertise and advice in the management of complex cases, palliative care, and safeguarding, ensuring high standards of care at home.
  • Ensure all team members are clinically competent, well-supported, and working within professional guidelines.
  • Maintain oversight of record-keeping standards, including medication charts and documentation audits, ensuring compliance with NMC and organisational policies.
  • Work closely with the Hospital Discharge Team and wider system partners to support safe, timely discharges and reduce avoidable hospital admissions.
  • Promote and embed clinical supervision, staff development, and evidence-based practice across the team.
  • Support and lead service development, using audits and performance data to drive improvements and meet local and national targets.
  • Manage complaints, incidents, risks, and staff concerns, escalating appropriately and ensuring lessons are learned.
  • A grounded yet forward-thinking nursing professional with a commitment to continuous improvement.
  • Strong communication skills and the ability to positively represent the Trust.
  • A role model who supports lifelong learning and contributes to the teams development.
