Mid Norfolk Primary Care Network (PCN) has an opportunity for an experienced clinical pharmacist, on a full time basis, to join their growing team, supporting six GP practices across the Dereham area.
The main responsibilities include:
- Manage the prescription request workflow.
- Manage complex face to face medication reviews with an emphasis on seeing patients with polypharmacy, deprescribing, and opiate prescribing.
- Manage network quality activities such as sharing best practice and cross-network audit.
- Develop and manage medicines optimisation services within the network practices.
- Provide leadership on quality improvement and clinical audit as well as managing aspects of medicines safety and antibiotic stewardship.
- Support the management of patients with long-term conditions.
- Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring.
- Work with care home staff to improve safety of medicine ordering and administration.
You will be a qualified pharmacist with strong experience within primary care. See below qualifications required.
Essential
- Minimum of 2 years (or equivalent) post qualifying experience as a pharmacist.
- Completion of an undergraduate degree in pharmacy.
- Working towards an independent prescribing qualification.
- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of patients.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
- Excellent IT skills.
- Ability to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Work effectively independently and as a team member.
Desirable
- Holding an independent prescribing qualification.
- Clinical Diploma.
