An exciting opportunity has arisen to join the Dermatology team in MCHFT as an Advanced Clinical Practitioner in Chronic Diseases. We are seeking a highly motivated and experienced nurse who will embrace this dynamic and challenging role. This role will be predominantly based at Macclesfield Health Hub, with some sessions also across Northwich and Crewe.
The team treat patients across the life span with acute and long term skin conditions, including those with chronic inflammatory skin disease. The ACP will work across sites, taking responsibility for a number of specialised clinics, to assess, diagnose and treat out-patients with a wide range of skin conditions including the prescribing of topical, systemic and biologic medicine.
They will work autonomously at an advanced level and use their expert knowledge and skills to make safe and effective clinical decisions, and to provide expert advice to patients, carers, peers and the wider hospital on a daily basis – both at the point of diagnosis and throughout the patient pathway. They will be committed to providing high quality and patient focused care at all times.
As a senior member of the dermatology department they will work effectively as part of the multidisciplinary team, demonstrating effective leadership and acting as a positive role model for others. They will be responsible for a complex caseload of patients and balancing this with conflicting non-clinical demands on a regular basis, to ensure all four pillars of Advanced Practice are met within their role.
The post holder will utilise skills of clinical expertise, leadership, research, education and management. Excellent and professional communication skills are vital. They will lead others, think strategically and communicate effectively. They will work collaboratively, influence decisions and outcomes within the department, and be committed to improving care in a complex healthcare organisation by implementing and evaluating evidence based standards, guidelines, procedures and policies.
They will be responsible for developing and auditing the ANP service within Dermatology, in addition to creating and developing specialist dermatology education to a wide range of individuals. The applicant must be a competent individual able to demonstrate self-awareness and integrity in post.
