An exciting opportunity for a registered nurse to join our forward‑thinking and award‑winning Care Community Team in Winsford, covering a 7‑day service.

Core Responsibilities

  • Assess nursing care needs, implement and evaluate care programmes, deliver high quality holistic care within the community and wound care clinics.
  • Support peers, the team leader and the organisation in promoting the Trust’s values and behaviours.
  • Work independently, manage time and organise work efficiently, and be flexible to work in other areas to meet service needs.
  • Travel is essential; candidates must demonstrate how they will meet this requirement.
  • Assess, plan, implement and evaluate care plans to meet patients’ physical, social, emotional and environmental needs, following district nursing contract.
  • Provide skilled nursing care including wound care, leg‑ulcer assessment and management, administration of medication via syringe pump, use of intravenous lines, venepuncture, and catheter care.
  • Interpret and analyse clinical data, record all patient contacts accurately on Emis Web and Trust documentation, maintaining confidentiality in line with NMC guidelines.
  • Promote evidence‑based practice, comply with Trust policies and maintain clinical competencies.
  • Work within the End of Life strategy, act as a clinical expert in EOL care provision, and practice safe use of clinical tools.
  • Deliver health education, promote self‑care and provide both planned and opportunistic care to individuals and communities.
  • Refer patients to other practitioners when risks or needs exceed scope of practice.
  • Prioritise and manage own caseload, delegate work appropriately and support colleagues.
  • Participate in quality monitoring, incident reporting, and off‑duty leave coordination.

Qualifications & Experience

  • Band 5 registered nurse.
  • Excellent communication, interpersonal and organisational skills.
  • Sound knowledge of recent NHS changes and political drivers.
  • Good clinical skill set, modern IT software proficiency.
  • Ability to work independently, innovative, flexible and adaptable.
