We are looking for a temporary Band 5 registered nurse to join our forward thinking, Gold Award winning Community Team in Nantwich and Rural. The role requires 30 hours per week (4 × 7.5 hour shifts) covering a 7-day service. Travel is essential; you must hold a full UK driving licence and have access to a car.

Key Responsibilities

  • Assess and manage nursing care needs and implement and evaluate care programmes.
  • Deliver high-quality holistic care within the community setting and wound care clinics.
  • Support peers, team leader and the organisation in promoting Trust values and behaviours.
  • Work independently, plan, organise and manage time effectively, and be flexible to work in other areas if required.
  • Maintain accurate recording of patient contacts on EMIS Web and Trust documentation.
  • Participate in health education and self-care promotion for patients and the community.
  • Ensure equipment and aids meet patient needs, are maintained and demonstrate safe use.
  • Provide skilled nursing care and clinical tasks such as wound care, leg ulcer assessment/management, medication via syringe pump, IV lines, venipuncture, catheter care.
  • Assist in end-of-life care, adhere to EOL tools, act as clinical expert.
  • Collaborate with healthcare professionals and agencies for service delivery.
  • Maintain clinical competence and participate in appraisal and clinical supervision.
  • Participate in quality monitoring and reporting of incidents/accidents.

Qualifications and Requirements

  • Registered nurse, Band 5 (temporary).
  • Good knowledge of recent NHS changes and current political drivers for community care.
  • Good clinical skill set, modern IT software knowledge.
  • Full UK driving licence and willingness to travel.
  • Strong interpersonal and communication skills, adaptable, reliable and approachable.
  • Good time-management, organisational, innovative and flexible.
