A Vacancy at Mid Cheshire Hospitals NHS Foundation Trust.

We are looking for an experienced Band 3 HCA to work within a busy community team. The post is based in Sandbach, working within SMASH care community.

The role includes caring for patients who are approaching the end of life. The post holder will undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency.

You will be working without close supervision, and will therefore need to use your own initiative, whilst following the patient\’s care plan. You must also be passionate about making a difference to people’s lives, helping them to live as well as possible and to die with dignity.

As well as being able to work independently, you will possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas – if required – to meet service need.

To join our dedicated, caring team, you will need excellent communication skills and emotional strength. We will expect you to have a talent for forging strong empathetic relationships with a variety of people, whilst providing palliative care. We will of course provide excellent on‑going training and support.

Key responsibilities

  • Deliver nursing interventions to patients in their own homes, including wound care, phlebotomy, medication administration and risk assessments.
  • Support the multi‑disciplinary team, maintaining clear, concise and legible documentation adhering to prescribed standards and protocols.
  • Use IT systems for relevant tasks such as accessing patient records & results, data collection and inputting data.
  • Obtain consent in accordance with Trust Consent Policy.
  • Report accidents/incidents on the clinical incident reporting system when they occur whilst on duty.
  • Maintain a patient‑orientated approach to care delivery, promoting a rehabilitative and self‑care approach.
  • Undertake phlebotomy duties as delegated by the registered nurse, working within policies and procedures of the Trust.
  • Undertake clinical tasks as delegated by the Community Nurse team and within sphere of competency.
  • Assist the Community Nursing Team in management of patients with long‑term conditions and in end‑of‑life care.
  • Recognise the need for referral and liaise with other health care and multi‑agency workers as necessary.
  • Participate in induction and orientation programmes for all grades of staff who require community experience as arranged by the Team Leader.
  • Learn and maintain up‑to‑date knowledge of blood test procedures, selecting appropriate sample tubes and ensuring correct labelling at the time of sampling.
  • Maintain phlebotomy stocks, keeping them clean and in good order.
  • Follow Trust policy for needle‑stick incidents, ensuring accurate reporting on the Ulysses system, and dispose of all syringes, needles and related items in accordance with health and safety procedures.

Your role will include the responsibilities listed above, and may require you to work across the footprint of Central Cheshire Integrated Care Partnership.
