Hospital@Home service is looking to recruit an enthusiastic and experienced registered nurse to join their team based at Broomfield Hospital

This is a highly visible role providing strong professional and clinical leadership within the Hospital@Home (H@H) team. The successful candidate will assist with supporting the sub-acutely unwell patient by providing care in the patient\'s home, administering IV medications, wound care management, supporting admission avoidance, triage and management of the patient in the community setting.

Main duties of the job

If you enjoy holistic one to one care, with the time to really support your patients, then this is the job for you.

The role is predominantly based in the community (although there will be some hospital site working), therefore you will need to have a full driving license and have access to a vehicle that can be used for work purposes.

You must be a registered nurse (NMC) as you will be administering Intravenous medications.

Main duties to include but not limited to:

  • Administration of IV medications in the patients home via Peripheral cannula or PICC line.
  • Supporting admission avoidance
  • Assessment and review of treatments
  • Support patients through their treatment via agreed clinical pathways
  • Review care plans and escalate concerns appropriately
  • Deputise for the virtual ward manager in their absence
  • Any other duties associated with the Hospital @ Home service.

Hospital@Home operates across all three sites within the Mid & South Essex University Hospitals Group and you will need to have a driving license and access to a car in order to perform this role. Work travel expenses are paid.

To ensure effective operation of the service the role will see the registered nurse safely administer a range of therapeutic interventions relevant as per agreed clinical pathways within scope of practice, devise relevant care plans and ensure that these are adhered to and kept current within the Hospital @ Home service.

To work within the senior nursing team in the Hospital @ Home team, managing a clinical caseload when the need requires, to facilitate supporting the Hospital @ Home service and to facilitate patients flow through identified pathways, including discharge planning.

Perform and support staff to complete holistic assessment and reassessment of all health and wellbeing needs.

Nursing needs: ensuring that every patient has an overview of health and nursing needs reviewed and recorded
