The Assessment & Referral Counselor is responsible for:
- Incoming inquiry calls and walk ins
- Crisis Intervention
- Triage as needed
- Completes assessments and provides referrals if necessary
- Interface closely with Medical Staff, Treatment Team, and Managed Care Organizations

Qualifications Requirements:
- Master's Degree in Social Work or related field; or be a Registered Nurse licensed in the State of Indiana
