This is an exciting opportunity for a Transitions Co-ordinator to join Metropolitan Thames Valley's contract team. The role is based in our Nottingham office. The Transitions Team provide expertise, programme management, governance, and support, to ensure the effective and smooth delivery of care and support contract implementation and service mobilisation, contractual variations, and change of building use.

As a Transition Co-ordinator you will:

  • Work with internal and external stakeholders supporting property evaluations, service mobilisations, change of use, service closures and new property agreements. You will need to build strong working relationships to deliver these projects effectively.
  • Support the Transitions Manager to ensure the effective delivery of various projects, whilst also leading on your own portfolio of work.
  • Ensure that the programme documentation is maintained, managed and updated.
  • Monitor and provide reports relating to performance data.

What you will need to succeed:

If you understand services delivered in Care & Support or Housing and are looking to develop your career this could be the opportunity for you. You will be responsible for your own workload and should be able to demonstrate managing and supporting numerous projects at any one time. The successful applicant will be required to communicate effectively with internal and external stakeholders both verbally and in writing. You will also be required to interpret complex written and numerical information so should have good literacy and numeracy skills.

Ideally, we are looking for candidates with a background in social housing; however, we recognise that skills are transferrable and welcome applications from candidates who are keen to learn and develop their skills and knowledge in a fast-paced environment.
