Role Summary

We are looking for a skilled and compassionate Diabetes Specialist Nurse to join our clinic.

You will lead nurse-led clinics and manage a caseload of patients with Type 1, Type 2, and complex diabetes, working within scope of practice and agreed clinical protocols.

You will deliver evidence-based care with a strong focus on patient education, diabetes technology, and lifestyle optimisation.

This is an autonomous role that requires sound clinical judgement, excellent communication, and a genuine commitment to patient-centred care.

  • Core Responsibilities
  • 1. Clinical Care and Case Management
  • Conduct comprehensive, holistic assessments of new and returning patients.
  • Design and implement personalised diabetes care plans
  • Manage and interpret data from Continuous Glucose Monitors (CGMs) and Flash Glucose Monitors such as Free Style Libre and Dexcom.
  • Assess, troubleshoot, and educate patients on insulin pump therapy where applicable.
  • Screen for and proactively manage diabetes-related complications including cardiovascular risks, neuropathy, and foot care.
  • Monitor blood glucose, weight, blood pressure, and other relevant health metrics to identify trends and inform treatment adjustments.
  • Provide regular updates to the Clinical Lead on clinic flow, patient escalations, and any deviations from clinical protocols.
  • 2. Patient Education and Empowerment
  • Deliver tailored, one-to-one education on carbohydrate counting, sick-day rules, and hypoglycaemia and hyperglycaemia management.
  • Provide ongoing emotional support and motivational interviewing to help patients take ownership of their self-management.
  • Advise patients on diet, exercise, and lifestyle changes to improve glycaemic control.
  • Educate patients on the clinic's care programmes, lifestyle interventions, and digital health tools.
  • Produce and send accurate, timely visit summaries to patients following each appointment.
  • 3. Data Collection and Digital Health
  • Collect and record patient health data accurately in the clinic's electronic patient record (EPR) system.
  • Analyse patient data trends to support early identification of complications and inform treatment decisions.
  • Support the integration of new clinical workflows and digital platforms into day-to-day practice.
  • Stay current with the clinic's evolving care models, patient programmes, and wearable technology.
  • 4. Infection Control and Compliance
  • Adhere to infection prevention and control protocols in line with NHS England guidance and the Health and Safety at Work Act 1974.
  • Ensure compliance with clinic policies, medication safety standards, and quality control procedures.
  • Follow safe handling and disposal procedures for medical supplies, sharps, and biohazard materials in line with UK regulations.
  • Report any clinical incidents, near‑misses, or patient concerns to the Clinical Lead in accordance with the clinic's incident reporting framework and CQC standards.
  • 5. Quality and Governance
  • Participate in regular clinical audits and support data collection for departmental reviews, including medication fridge logs, equipment calibration checks, and stock counts.
  • Actively contribute to CQC compliance efforts and adhere to the clinic's quality improvement and patient safety frameworks.
  • Participate in annual nursing competency assessments and skills validation reviews.
  • Contribute to risk assessments, policy reviews, and complaints learning processes.
  • Adhere to the Duty of Candour in line with CQC requirements and NMC guidance.
  • Maintain accurate, confidential patient records in line with NMC guidelines, UK GDPR, and the Data Protection Act 2018.
  • 6. Interdisciplinary Collaboration
  • Work closely with Consultant Endocrinologists, Dietitians, General Practitioners, and lifestyle coaches to deliver a joined‑up patient experience.
  • Actively participate in team meetings, case discussions, and quality improvement initiatives.
  • Provide compassionate support to patients and their families throughout the care journey.
  • Key Requirements and Skills

Experience

  • Proven post‑registration experience in a relevant clinical setting with a strong background in diabetes nursing.
  • A minimum of two years of clinical nursing experience in endocrinology, diabetes care, or chronic disease management is preferred.
  • Experience with digital health tools, wearable devices, or patient education programmes is an advantage.

Education

  • Bachelor's Degree or Diploma in Nursing from a recognised UK institution.
  • Post‑graduate qualification in Diabetes Care or Management, such as the Warwick Diabetes Certificate or equivalent, is highly desirable.
  • Registration
  • Current, active registration as a Registered Nurse (Adult) with the Nursing and Midwifery Council (NMC).
  • Prescribing
  • Non‑Medical Prescriber (V300) qualification is a distinct advantage. Willingness to complete the training will also be considered.

Certifications

Immediate Life Support (ILS) or Advanced Life Support (ALS) certification is preferred.

Basic Life Support (BLS) certificate is required.

This role operates within the regulatory framework of the Nursing and Midwifery Council (NMC) Code of Professional Conduct.

The post‑holder will work in accordance with Care Quality Commission (CQC) standards, the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, and all relevant UK employment legislation including the Equality Act 2010.

Patient information must be handled in full compliance with UK GDPR and the Data Protection Act 2018.
