We are seeking exceptional Social Workers for our Targeted Review Team. Are you passionate about supporting people to live independent, fulfilling lives? Do you have the skills and compassion to ensure care services truly meet the needs of adults in our community?
We’re looking for dedicated professionals to join our Adult Social Care Targeted Review Team – a dynamic, person-centred team committed to reviewing, improving, and tailoring care packages that empower individuals while promoting dignity, choice and control, and independence.
The Team will consist of a Team Manager, Social Workers and a Social Work Assistant who will carry out Care Act reviews and assessments on targeted residents to maximise independence and ensure choice and control. You will require experience and knowledge of Adult and Health social care legislation and guidance to carry out assessments, reviews, support planning and safeguarding.
