Responsibilities
- To safeguard vulnerable adults in need of support who are referred to the department, ensuring that plans are put in place which will mitigate risks.
- To obtain an accurate, person centered perspective of the adults needs in order to ensure that the most appropriate support is provided to achieve their optimal independence.
- To work collaboratively with Adults and their carers who are experiencing particularly complex issues and assist them in finding solutions.
