The Community Occupational Therapists role is to promote optimum independence through the process of a specialist assessment incorporating policies and guidelines to which the department works towards and the criteria for the provision of specialist equipment and adaptations to enable individuals to continue to live in the community and support informal and formal care providers.
In addition, to be able to provide advice and support rehabilitation techniques to help maintain or increase level of independence (where appropriate). As well as liaising and collaboratively working with other professionals and agencies with regard to the individual and make ongoing referrals if appropriate.
The nature of this post is primarily to collaboratively work alongside our Housing Solutions team delivering a Community Occupational Therapy response/approach in ensuring service users/members of the public are assessed and responded to within an expected timeframe and standard. Whilst the primary function of the role is to work collaboratively and alongside our housing colleagues, secondary to this role is promoting optimum independence.
You will be required to provide evidence of all qualifications specified as essential. Ability to speak Welsh is desirable.
