Responsibilities
- To monitor Domiciliary Care Service Contracts, Supported Living and other Agreements with all service providers within Adult Services.
- To provide a contract monitoring service that ensures Providers of adult social care deliver services in compliance with the contract service specification.
- To evaluate the performance information relating to adult social care providers to support the development of services in Merthyr Tydfil.
- To facilitate contracts for care services at individual service level, with the most appropriate provider. To manage the flow of home care hours to approved providers ensuring that services are commissioned that offer best value for money.
- To set up contracts quickly to move people on from short-term services and minimize delays to hospital discharges.
- To control demand for home care by managing requests, acting as the central point of contact for providers and coordinating the response, including appropriate authorization.
- To maintain robust records and management information on contracted services.
