An opportunity for an experienced Occupational Therapist to join the St Helens 2‑hour Urgent Crisis Response (UCR), Community Frailty and Virtual Ward Therapy Team. The role is predominantly community based, supporting adults in crisis with health or social care needs at home and within the community.
Key responsibilities:
- Provide urgent and timely response to referrals 7 days a week, 8 am‑8 pm.
- Conduct rapid assessment and deliver community intervention for up to 72 hours.
- Process referrals, manage a caseload independently and independently assess and treat patients with a variety of medical conditions.
- Deliver specialist rapid assessment and interventions to support patients to remain safely in their own homes and prevent unnecessary A&E attendances.
- Work collaboratively within a multi‑disciplinary team comprising Frailty Consultants, Advanced Clinical Practitioners, Occupational Therapists, Physiotherapists, Nurses, Social Workers and support workers.
- Champion an integrated community care approach, prioritise workload and adapt to changing circumstances.
Qualifications:
- Degree in Occupational Therapy (or equivalent).
- HCPC registration.
- Clinical Educators Certificate.
- Post‑graduate study (e.g., MSc in relevant health studies) is desirable.
Experience:
- Advanced clinical knowledge and evidence‑based practice in a relevant healthcare setting.
- Experience of line‑managing a team, setting objectives, conducting appraisals, effective delegation and coordination.
- Demonstrated effective working within an MDT.
- Experience of service development, problem‑solving approaches and continuous professional development.
- Audit and research experience in a clinical area.
Skills:
- Strong team motivation and performance facilitation.
- Excellent prioritisation, deadline management and effective communication.
- Accurate record keeping, report writing and IT skills including presentation abilities.
- Flexible approach to working with good time management.
Additional responsibilities:
- Ability to travel to various community locations and hospital sites using own car or assisted driver.
