We are seeking enthusiastic and motivated Registered Nurses to join our Community Nursing Team. Do you have a passion for delivering outstanding patient care that brings the 6 C's (Care, Compassion, Commitment, Courage, Communication and Competence) to life? If so, we want to hear from you. Community nursing is located across multiple sites in the borough of St Helens. We provide treatment, support and guidance for a wide range of health issues within patients' own homes.

An Enhanced DBS Certificate, with the checking of both the Children's and Adults' Barred Lists is required for this post.

Main duties of the job include:

  • To establish, maintain and develop professional working relationships with colleagues
  • Follow all agreed Quality Assurance Mechanisms in operation and contribute generally to the establishment and development of a quality service
  • Adhering to policies and procedures including standing financial instructions and orders
  • Participate in the Trust's Development Review Appraisal process and, with line management, objectives and development plans
  • All staff are entitled to participate in the Review Process and, with their manager, are jointly responsible for the completion of agreed actions
  • Comply with the Health and Safety at Work Act 1974
  • Be aware of, and responsible to, the changing nature of the Trust and adopt a flexible and pro-active approach to work
  • Contribute to the health promotion strategies both internally and externally
  • Be familiar with the arrangements for safeguarding children, young people and vulnerable adults
  • Ensure safeguarding and promoting the welfare of children, young people and vulnerable adults forms an integral part of their duties
  • Attend mandatory training on infection prevention and control

Clinical and professional responsibilities include:

  • Adhere to relevant code of practice of professional body
  • Maintain and utilise advanced clinical skills
  • Implement nursing care as per care plan
  • Develop competency in wound care

Administrative responsibilities include:

  • Ensure patient records are kept up to date and accurate

Teaching and training responsibilities include:

  • Support the development of junior staff through clinical supervision and mentoring
  • Attend mandatory training including safeguarding
  • Demonstrate commitment to personal continuing education

Research and audit responsibilities include:

  • Participate in research
  • Assist in the Department's Audit Programme

Line management/supervisory responsibilities include:

  • Act as preceptor/mentor to new staff and to pre-registration nurses and students

Qualifications required:

  • 1st Level Registered Nurse (degree/diploma)
  • Current NMC Registration
  • Evidence of continuing professional development
  • Knowledge of NMC professional standards for practice contained within the code
  • Experience of working in a clinical environment
  • Understand requirements for NMC professional Revalidation
  • Experience of working within a multi-disciplinary team
  • An understanding of national, regional and local NHS issues
  • Experience of providing and receiving complex, sensitive information

Skills required:

  • Excellent communication skills with the ability to network and build relationships at all levels
  • Effectively prioritises and co-ordinates own workload
  • Ability to de-escalate complex situations
  • Ability to communicate effectively with patients/relatives and carers and all members of the multi-disciplinary team
  • Aware of the need for confidentiality at all times
  • Computer literacy e.g. experience of electronic patient record

Other requirements include:

  • Frequent moderate effort for several short periods of time such as manual handling of patients and equipment
  • Frequent concentration required for drug calculation and administration
  • Ability to work within highly distressing or emotional circumstances e.g. end of life care
  • Ability to communicate distressing information
  • Ability to communicate in a challenging environment
  • Able to carry out direct clinical care as required on ward or department
