The predicted salary is between 28800 - 43200 £ per year.

An exciting opportunity has arisen for an Assistant Practitioner to join the St Helens Early Supported Discharge team. The service provides stroke rehabilitation to the people of St Helens in their own homes in order to gain function and independence after a stroke.

Additional funding has been agreed for the next 12 months to expand and develop the service on offer. The successful candidate will be involved with supporting this pilot project giving plenty of opportunity to develop neuro and leadership skills alike. In the event that funding is not continued after the initial pilot, the successful candidate will be offered an alternative post within the organisation.

This is a community based post, and the ability to travel between visits is essential., To provide specialist and skilled assistance in the delivery of Physiotherapy / Occupational Therapy / Rehabilitation for adults referred in to the. St Helens Community Therapy services.To plan, review and independently implement specialist rehabilitation and enablement programs for a variety of clients in varying settings, within a defined area of competence.To work closely with multidisciplinary teams and other professionals, particularly in relation to patient mobility and enablement, including assessment for and provision of appropriate equipment, and to assist in the development of good working relationships with statutory, voluntary, and independent organisations.To contribute to the development of this role within the Community Unit, utilising highly developed skills and experience.
