This is an exciting opportunity for experienced health care staff to join the team in Longmoor House. The Reablement Worker will function as part of the multidisciplinary team within a sub‑acute intermediate care setting to provide a seamless service to patients.
The post holder will continue to develop and undertake a range of delegated tasks and will report to a registered Nurse or Allied Health Professional. The Reablement Worker will provide general/specific rehabilitation, reablement and personal care for a patient group and will be proficient and competent to work across professional disciplines. The Reablement Worker will also be supported to achieve competencies to enable them to work in‑between the disciplines.
Successful candidates will be expected to work across a 24/7 shift pattern and will have the opportunity to learn, develop and practice a variety of skills, as well as being a valued member of our exceptional multi‑disciplinary team.
