Overview

Enhanced Nurse Practitioner required at Mersey Care NHS Foundation Trust. The role sits within Integrated Community Reablement Assessment Service (ICRAS) to support admission avoidance, early supported discharge and home-based care.

Job Summary

Work alongside Advanced Clinical Practitioners to provide a 2-hour response to assess patients to enable management at home and prevent hospital attendance. Provide clinical support to ICRAS teams and collaborate with acute hospitals to enable early supported discharge and follow-up in patients\\\’ homes. The role will work across all Neighbourhood Teams.

Main duties

  • Lead and deliver high-quality care, including risk management, compliance with CQC requirements, incidents reporting and clinical audit.
  • Provide holistic assessments and case management for complex patients as part of the integrated community nursing service.
  • Identify risks to service delivery and allocate resources to prioritise care.
  • Ensure clinical research is conducted in line with Mersey Care NHS Foundation Trust protocols with ethical approvals where required.
  • Ensure patients receive information, are treated with dignity and respect, and care is patient-focused with emphasis on self-care and local population needs.
  • Attend statutory and mandatory training and develop learning opportunities for the team.
  • Act as a clinical expert and information resource, leading training, education and orientation for staff and students; mentor staff and students as required.
  • Develop, implement and monitor public health and health promotion activities in line with national and local priorities; integrate evidence-based findings into practice and participate in Trust-approved research programs.
  • Lead development, implementation and audit of protocols, policies and integrated pathways; input activity onto the Trust database; manage risk and liaise with colleagues to maintain service delivery.

Job Description & Responsibilities

To lead processes for delivering, monitoring and improving quality of care, including risk management and contributing to CQC requirements, incidents and clinical audits. Deliver holistic assessments, care planning and treatment for complex patients. Identify and mitigate risks to service delivery and ensure appropriate use of resources. Ensure ethical approvals for research; promote patient dignity and privacy; complete risk assessments and maintain safe systems of work. Monitor staff performance, provide supervision and development, and facilitate learning opportunities. Supports and challenges caseload management to ensure safe and effective care. Promote ongoing education and mandatory training. Lead in training, education and orientation of staff and students; act as mentor; drive public health activities; integrate evidence-based practice and research; input activity data for contractual requirements.
