We are an expanding leading home care provider in the South West and currently recruiting for a Care Coordinator to maintain and develop our home care services. Our Care team is friendly, and we love what we do. We are passionate about the high-quality support we provide and offer.
Core Duties & Responsibilities:
- Roster and Template Care Calls: Primary role is to roster and template care calls using an electronic system.
- Direct Management and Supervision: Oversee the care team, ensuring staff development through supervisions, appraisals, and training to assist them in delivering a high-quality service. Spot-check carers in the community, identifying good practice and reporting training concerns.
- Care Planning and Assessment: Creating, managing, and monitoring customer care plans, ensuring staff rotas meet service users’ needs and aspirations. Also, complete care assessments and writing up Care Plans.
- Compliance and Quality Control: Ensure that care assessments, care, and risk planning meet best practice and customer aspirations. Work to achieve the best care standards possible and comply with CQC regulations.
- Staffing: Work with the Recruitment Team to ensure sufficient current and future staffing levels.
- Emergency Support: Provide care services to service users in an emergency, arrange emergency cover when required, and cover all/any calls reasonably where required.
- Communication: Ensure that the telephones are answered promptly, and people are spoken to in a polite and respectful manner.
- Relationship Management: Build relationships with care staff and service users, and assist in preparing for inspections by the local authority and CQC.
- On-Call Duty: Participate in the paid on-call rota.
- Additional Support: Assist the registered manager and care coordinator with any duties that may be required.

We are looking for a Care Coordinator to join us who is:

- Mobile: Full Driving Licence and access to a vehicle (essential).
- Qualified: NVQ Level 3 in Health & Social Care (Essential).
