Work with us as a Health and Social Care Technician in a dynamic multidisciplinary team who are having an incredible impact providing community care and support in Hammersmith and Fulham. The Community Independence Service is co‑located with LBHF Adult Social Care on King St, Hammersmith. Our service works closely with local GPs, care homes, social services and secondary care partners, such as Imperial College Healthcare NHS Trust sites and Chelsea & Westminster Hospital NHS Foundation Trust. You will be connected with peers across the CIS tri‑borough of Hammersmith & Fulham, Westminster and Kensington and Chelsea as well as West London NHS Trust.

Working as a member of the Community Independence Service / Case Management Team you will be supported by registered allied health clinicians and ICAP (Integrated Care Access Point). You will be able to deliver skilled health and social care to people in their homes; the care you deliver will be based on delegated tasks by a senior clinician following thorough holistic assessment of person’s clinical and personal needs and their agreement of an individualised care plan.

The care you deliver will be within the boundaries of your role, focusing on supporting people with a range of complex needs and long term medical conditions (mental and physical). The aim is to reduce the impact of health inequalities, improve access to health and care services, address complex health, social and housing needs, reduce social isolation and optimise wellbeing.
