Your role
Clinical Pharmacist – Full-time position, 37.5 hours per week. Ideally an Independent Prescriber with previous Primary Care experience and completion of, or currently undertaking, the CPPE Primary Care pathway. Duties include Structured Medication Reviews, polypharmacy management, clinical audits, repeat prescribing, medication reviews, discharge letter processing, and support with QOF and IIF targets. Predominantly telephone-based clinics. Work collaboratively alongside an experienced team of Clinical Pharmacists within the PCN. Encouraged and supported to develop specialist interests. Deliver high-quality, patient-centred care throughout.
