Menlo Park Adult Social Care is seeking a General Practitioner to join a care home hub in Weston-super-Mare. You will hold a caseload, deliver proactive assessments, care planning and ensure continuity for residents, families and staff.
You will work as a senior clinician within a multidisciplinary team, supporting advanced practitioners, pharmacists and nurses, with 4–6 sessions over 2–4 days and some remote work.
