Salary: £16,914 – £19,701 per annum pro rata

As a Service Support Worker you will provide reactive, focused and flexible support to adults following a hospital stay, enabling early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinator by assessing service users’ needs, completing and following a support plan, providing practical and emotional support in the service user’s own home, and signposting to other services where appropriate to enable independent living.

Responsibilities
  • Assess service users’ needs and complete support plans.
  • Respond to referrals from clinicians and health professionals.
  • Provide practical and emotional support in the service user’s home.
  • Coordinate with external services to maintain a network across health, social care and emergency services.
  • Maintain the quality of service and contribute to continuous improvement.
Qualifications and Experience
  • Education to GCSE level (or equivalent by experience).
  • IT literate.
  • Full manual‑transmission driving licence holder with access to a vehicle (required to obtain business‑use vehicle insurance).
  • Knowledge of services provided by the NHS and social care.
  • Professional, diplomatic, and confidential communication skills.
  • Flexibility to work outside normal hours.
Additional Requirements
  • Use of full personal protective equipment, including fluid‑resistant face masks, at all times.
  • Completion of a risk assessment to confirm suitability for frontline work at onboarding.
  • Full vaccination against COVID‑19 with a UK‑approved vaccine (evidence of status or exemption required).
