As a Service Support Worker you will provide reactive, focused and flexible support to adults following a hospital stay, enabling early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinator by assessing service users’ needs, completing and following a support plan, providing practical and emotional support in the service user’s own home, and signposting to other services where appropriate to enable independent living.
Responsibilities- Assess service users’ needs and complete support plans.
- Respond to referrals from clinicians and health professionals.
- Provide practical and emotional support in the service user’s home.
- Coordinate with external services to maintain a network across health, social care and emergency services.
- Maintain the quality of service and contribute to continuous improvement.
- Education to GCSE level (or equivalent by experience).
- IT literate.
- Full manual‑transmission driving licence holder with access to a vehicle (required to obtain business‑use vehicle insurance).
- Knowledge of services provided by the NHS and social care.
- Professional, diplomatic, and confidential communication skills.
- Flexibility to work outside normal hours.
- Use of full personal protective equipment, including fluid‑resistant face masks, at all times.
- Completion of a risk assessment to confirm suitability for frontline work at onboarding.
- Full vaccination against COVID‑19 with a UK‑approved vaccine (evidence of status or exemption required).
- 36 days holiday (including bank holidays) per annum, pro‑rata.
- Option to purchase an extra 5 days annual leave.
