Meeveem is seeking a qualified Social Worker to join the Leaving Care Team in Huntingdon.
You will oversee assessments and Pathway Plans for care-experienced young people aged 16–18, coordinating with schools, health services, housing and education providers to support safe transitions to adulthood.
You will build strong relationships, maintain thorough case recordings, and contribute to a proactive duty system while safeguarding and promoting welfare at every step.
