Community Learning Disability Nurse - Adults in Hollingworth

To work within the Learning Disability Directorate as part of a multi-disciplinary integrated team. The post holder will provide specialist health and behaviour assessments to individuals referred to the learning disability team. The post holder will be required to undertake positive behaviour support training if they do not possess this qualification. Interventions will be delivered across different settings including the person's home. There will be a requirement to liaise with partners and stakeholders to ensure responsive, effective, safe and person-centred services are delivered.

Main duties of the job

• At all times to work in accordance with Pennine Care NHS Foundation Trust policies and procedures.
• The post holder will work in partnership with all members of the Multidisciplinary Team to achieve the 'Good Health' objectives of 'Valuing People'.
• To provide a safe, effective, responsive, efficient, timely, community nursing service to people with learning disabilities, their families and carers.
• To undertake a range of comprehensive assessments for individuals with a learning disability and complex and challenging needs.
• This assessment work requires a specialist knowledge base on which to practice.
• To monitor and evaluate complex interventions and packages of care.
• The post holder may be required to become a physical intervention trainer, this task requires the trainer to have highly developed and co-ordinated physical skills.
• The post holder will be expected to offer behaviour support training to identified others including teams around the adult.
• To develop a range of interventions and specialised care plans.
• To be responsible for co-ordinating the provision of clinical care, within a defined caseload accessing supervision from a designated nurse as appropriate.
• To embrace the public health role, promoting health and well-being and where possible, prevent ill health.

Job responsibilities

• To undertake any other reasonable duty, which is appropriate to the band, when requested by Senior Staff.

• To support the work of the Community learning disability nurses, in reducing the health inequalities faced by people with learning disabilities.
• To work collaboratively with primary care and secondary care services to ensure the health needs of people with learning disabilities are met.
• The post holder will undertake an active role in identifying and managing risk.
• The post holder may be required to participate in Safeguarding Adult investigations.
• The post holder will be required to work in partnership with Social Services and Child Safeguarding.
• The post holder will receive appropriate support to manage their own caseload and will be expected to prioritise competing demands.
• To work in partnership to achieve planned co-ordinated discharges.

Person Specification

Education / Qualifications

• Essential: Registered Nurses Learning Disabilities (RNLD) Post registration training / willingness to undertake training plus experience to postgraduate diploma level.
• Desirable: Clinical Supervision / Mentorship qualification.

Experience

• Essential: Demonstrate knowledge and experience in supporting individuals who have a learning disability. Experienced in delivering advice and support to a wide range of people. Experienced in partnership working. Experienced in managing risk.
• Desirable: Experienced in delivering training. Previous community nursing experience desirable. Experience in supporting individuals with complex health needs.

Knowledge

• Essential: Established knowledge of the health inequalities agenda for people with learning disabilities. Understanding of Person Centred Planning. Evidence of continued professional development. Knowledge of Child Safeguarding and Safeguarding Adult legislation. Knowledge of current strategic drivers for community learning disability teams.
• Desirable: Knowledge of the evidence based around people who present with offending behaviour. Completed in depth training in Person Centred Planning.

Skills and Abilities

• Essential: Ability to complete complex assessments. Highly developed communication skills to support a range of activities. Ability to collaborate in the completion of formulations. Ability to develop multi-element interventions / support packages. Ability to incorporate key principles of 'Valuing People' into direct client care. Ability to proactively manage competing demands.
