Occupational Therapist Clinical Specialist - Hospital Outreach Support Team (HOST)

We are searching for a dynamic and adaptable therapist to support individuals in achieving their functional goals at home following their discharge from hospital.

Main duties of the job:
Outreaching from the acute site, you will work alongside a supportive and skilled team of therapists, managing your own caseload while delivering outstanding patient care in patients' homes. The post holder assists in the development, administration and provision of a specialist Occupational Therapy service; carries a varied caseload of patients, managing complex assessments with predominantly physical conditions and with regard to the physical, emotional, psychosocial, spiritual and social aspects of their care.

Job responsibilities:
Directly responsible for the management of own caseload of patients with diverse presentations and complex physical and psychological conditions, referred for Occupational Therapy assessment, rehabilitation, treatment planning and discharge management. Assists in the management of the specialist team providing, developing and planning an Occupational Therapy service to patients for this client group. The post holder will act as a source of expertise, demonstrating and maintaining knowledge, skills and experience in this specialist role.
Person Specification:
Qualifications:
- Essential: Diploma/degree in OT by recognised awarding body e.g. Royal College of Occupational Therapists. Must satisfy the criteria for full membership of the Health and Care Professions Council (HCPC).
- Desirable: An approved Occupational Therapy supervisor for OT/PT degree students.
Knowledge:
- Essential: Knowledge of the role of Occupational Therapy for people with a wide range of physical dysfunction, previous experience relating to specialist area, experience of working in an acute hospital setting, knowledge of relevant NHS and SS legislation, experience with community/mental health conditions, used to working in a busy environment, working knowledge of Microsoft Office and any other relevant IT systems applicable to the applicant, experience of undertaking clinical supervision, experience of managing and motivating a team and reviewing performance of the individuals, thorough knowledge of the Inclusion (equality and diversity) agenda.
- Desirable: Experience of delivering teaching and training sessions.
