Join Somerset NHS Foundation Trust as a Band 6 Complex Care Nurse within our innovative Complex Care service. You will work across South Somerset, collaborating with GP practices and neighbourhood teams to provide proactive, personalised care for individuals with significant health and social needs. Main duties of the job

As a Complex Care Nurse, you will coordinate and deliver comprehensive care plans using a holistic approach, including comprehensive geriatric assessments and medication reviews. You will work closely with multidisciplinary teams across primary, secondary, and specialist care services to enable patients' self-management and ensure seamless care. Your role includes supporting patient education, safeguarding, clinical governance, and contributing to service development and quality improvement initiatives. You will also undertake home visits within rural settings, requiring flexibility and excellent communication skills.

Job responsibilities

This role involves leading clinical care within a multi-agency framework, supporting patients with complex health and social needs. You will develop, implement, and evaluate personalised care plans, liaising with GPs, specialists, social care, and voluntary services. Your responsibilities include delivering evidence-based nursing, promoting patient and public involvement, and maintaining confidentiality and data protection. You will engage in continuous professional development, clinical supervision, and contribute to research and service innovation. Qualifications

Essential

  • Registered General Nurse (Adult) with current NMC registration.
  • Minimum 3 years post-registration experience at Band 5 or above in complex care or related specialty.
  • Evidence of continuous professional development.

Desirable

  • Degree in nursing or working towards a degree pathway recognised mentoring qualification or prepared to work towards.

Experience

  • Demonstrable autonomous working and clinical decision-making skills.
  • Experience of multi-disciplinary team working and collaborative care planning.
  • Ability to conduct comprehensive holistic assessments including geriatric and medication reviews.
  • Evidence of good communication skills.
