You will work across South Somerset, collaborating with GP practices and neighbourhood teams to provide proactive, personalised care for individuals with significant health and social needs. Main duties of the job
As a Complex Care Nurse, you will coordinate and deliver comprehensive care plans using a holistic approach, including comprehensive geriatric assessments and medication reviews. You will work closely with multidisciplinary teams across primary, secondary, and specialist care services to enable patients' self-management and ensure seamless care. Your role includes supporting patient education, safeguarding, clinical governance, and contributing to service development and quality improvement initiatives.
