The role involves four sessions per week with Monday fixed and one other day (Tuesday/Thursday/Friday), plus a hybrid on-call/triage day every other week and 13 patient clinics (mix of face-to-face and telephone).
The team includes pharmacists, nurses, social prescribers and a mental health practitioner, supported by an admin team, with modern digital workflows.
