Job summary

This unique role will predominantly provide increased capacity and availability of access to biologic therapy for patients with complex, severe asthma. This is a dynamic service working as part of a joint working service with the specialist centre in Exeter.

The role will offer varied aspects of nursing care from autonomously running nurse-led clinics, inpatient and outpatient contact and cross centre working with a wide and varied multidisciplinary team, fully supported by senior colleagues including respiratory consultants and specialist nurses.

Main duties of the job

  • To provide high quality review of inpatients admitted with asthma ensuring assessment, planning, implementation and evaluation of individualised programmes of care in collaboration with the multi-disciplinary team in particular the Respiratory Virtual Ward
  • To provide nurse led clinics for the administration and monitoring severe asthma patients starting biologic therapy
  • To support the senior asthma nurse with the workup and referral of patients with Severe Asthma who require input from the Tertiary Complex Asthma Service
  • To work in collaboration with the Complex Asthma Service in Exeter, forming part of their multi-disciplinary team and supporting at severe asthma clinics when needed
  • To support transition services between paediatric and adult services to provide seamless change over of care to teenagers and young adults living with complex respiratory disease
  • To work with and support other members of the respiratory nursing team to provide cross cover support for patients with other respiratory conditions including COPD, Interstitial Lung Diseases, Bronchiectasis and TB
  • To have an awareness of the need for ongoing audit of service provision to ensure best evidence based clinical practice

Job responsibilities

  • Develops and maintains effective working relationships with a wide range of colleagues across all levels from other disciplines, both within and outside the trust including; Respiratory Consultants, physicians, physiologists, specialist nurses, practice nurses, community nursing teams, allied health professionals, Pharmacy, and service leads
  • Ability to provide and receive highly complex, sensitive information
  • Promotes and maintains positive and effective links of communication with patients, their carers and visitors to the Trust
  • Ensure systems are in place to support good communication, written and verbal, with colleagues and other members of multi-disciplinary team
  • Understands barriers to effective communication and implements appropriate skills to manage and improve this
  • Works as an advocate for patients and ensures their understanding of complex diagnoses treatment plans
  • Participates with the South West Asthma and National Asthma Nurse networks
  • Communicates effectively with colleagues from the Complex Asthma Service to ensure appropriate management of patients with severe asthma
  • Communicates appropriately and skilfully with teenagers and their parents and carers who are going through the transition process from paediatric to adult respiratory services
  • Communicates in a respectful and empathetic manner at all times
  • Works to build a strong rapport with patients living with long term conditions who may need to access the service for a number of years

Person Specification

Essential

  • Respiratory nursing experience
  • NMC registered
  • 2 years post reg experience

Desirable

  • Previous Band 6 experience
  • working towards degree/MSC
  • relevant respiratory course/communication
