Practice Nurse Tonbridge Primary Care Network Kent

We are seeking a motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, ANPs, and Care Coordinators, delivering high-quality, patient-centred care.

The successful candidate will play a key role in supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. You will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work with GPs and the multidisciplinary team to coordinate ongoing care and treatment. The role also involves delivering a range of nurse-led clinics and housebound and care home visits, including vaccinations and, where appropriately trained, procedures such as FeNO testing, ECGs, and ear irrigation.

We welcome applications from experienced Band 5 nurses developing within primary care, newly qualified nurses with a strong interest in community and frailty care, and Band 6 nurses seeking a varied and autonomous role. You will be confident working independently across the PCN, demonstrate excellent communication skills, and support multidisciplinary working to improve patient outcomes. About Tonbridge PCN: Serving approximately 70,000 patients; Tonbridge PCN strives to develop collaborative working across Tonbridge GP practices and local health and care services. Tonbridge recently won a Kent and Medway award for its Integrated Home Visiting Service, jointly staffed by PCN GPs, paramedics and local community nursing teams. The PCN aims to support and strengthen delivery of primary care, offering high standards of patient care centred on the needs of our population, whilst working to support a happy and resilient workforce through the development of robust multi-disciplinary teams.

Job responsibilities include:

• Support public health and screening programmes, including immunisations and vaccinations, where appropriate.
• Completing CGAs and Respect forms.
• Deliver expert nursing assessments and manage complex care needs of housebound and care home residents within Tonbridge PCN in partnership with GP.
• Support patient and public involvement to enhance patient care and service delivery.
• Deliver a range of nurse-led clinics and undertake housebound and care home visits, including providing vaccinations and, where appropriately trained, carrying out procedures such as FeNO testing, ECGs, ear irrigation, and other clinical interventions.
• Create a trusting partnership with patients and/or relatives/carers to communicate and explain complex medical issues.
• Mentor, support, and supervise new-to-general-practice nurses, student nurses, and other healthcare professionals across the PCN by agreement and when appropriate.
• Be proficient in use of digital technologies applicable in the workplace to enhance personalised care.
• Advocate for people and nursing.
• Undertake mandatory training and CPD to meet NMC revalidation and practice needs.
• Provide clinical support and guidance to care home staff to improve patient safety and health outcomes.
• Identify own personal development needs, proactively seek feedback, and take effective action to address them using a mix of strategies.
• Work collaboratively across the PCN, actively engaging with community services, secondary care, social care, and voluntary sector providers to ensure integrated and holistic care pathways.

• Adhere to CQC standards, infection prevention and control protocols, and safeguarding responsibilities.
• Participate in audits, significant event reviews, and quality improvement initiatives.

Qualifications:

• Degree in Nursing or Registered Nurse Degree
• Registered with Nursing and Midwifery Council
• More than one post-registration qualifications and CPD, for example certificates, diplomas or professional development courses at level 6 (e.g. long-term conditions care and/or public health initiatives)
